A failed axlebox on this wagon resulted in the derailment of 6M08 - the 0140 Haverton Hill to Glazebrook in Summit Tunnel on the 20th December 1984. The resulting fire raged uncontrollably for over 24 hours, and ended up fusing some of the wagons, tunnel lining and track together. Fortunately, due to the flames escaping through ventiltion shafts, no lives were lost.
